looks like NPC Trisha does not want us players to upgrade our gears
when i try upgrade my Primal helmet from +6 to +7
Trisha gives me an error 'This item is incorrect'
but i still have 10/60 upgrades left
also, i try to upgrade my other Primal armor pieces from 25/60 but all grey-out lol

How do you get 25/60 on primal? It should be 25/25 which is prob why it's greyed out. The only thing with over be 25 should be aov gear (40).
Yes, old items since the update does not exceed 25/25!

If you proc the bug once by enhancing an item beyond its previous enhancement cap you'll keep receiving that incorrect item error every single time you attempt an enhancement, until you restart your game client.

Wow, you're right, it appears everything (aov as well) is now 60!
There's no mention of this in the patch notes :
