Its additive, for exemple if you get 40% from equipement and 100 from premium exp book, you will get 140 in total

I'm not sure about number 3, I never tried it, but you can stack the Premium Collected Works with the Premium Battle Book for a 200% boost.

I would personally use those exp books at like level 55 or something. That's the real grind =w=

You can use multiple of different EXP items, they will all stack as long as you don't use 2 of the same book.
In KR/JP there were 3 types of EXP books, you could activate all 3 and get a total of like +180% exp from them.
